As Henley's lines suggest, the Warrior tradition of pride in a performance that is
courageously and skillfully executed is an inextricable part of Warrior Athletics.

Through our athletic programs, we seek to provide opportunities for:
  • The development of physical competency.
  • The demonstratin of leadership.
  • The development of a clearer understanding that it is essential for human beings to work together.
  • The development of respect for diversity.


Parents:  To ensure your child's eligibility, do not turn in forms to our Main Office or Athletic Director's office. 

You must hand the physical papers, which include the physical form, the consent form and the concussion form, to their child.  Have the child walk to the Athletic Training Room after school and hand all the forms to an athletic trainer.  If the forms are incomplete, the forms will be returned to the child for completion.  The child will not be able to participate until all forms are completed and stored in the athletic training room.

Faxing of Forms:

If at all possible, forms should be delivered directly to the Athletic Training Room Staff after the final bell of a school day.  Forms can be immediately reviewed and handed back if incomplete.  Coaches and School Office personnel cannot accept completed forms.  If forms cannot be delivered directly to the Athletic Training Room, an alternative has been set up.

A medical doctor's office may now directly FAX forms to the Athletic Training Room under the following conditions: 1.  all forms, including a completed Physical Exam, Consent and Concussion form must be included in the transfer, 2. all forms must originate from the doctor's office.  Any incomplete form will make the student-athlete ineligible for participation until completed. We cannot accept FAX documents directly from a student athlete, nor can we accept photocopied completed forms.  These forms are legal documents, so must have original signatures.
